Use

How do I schedule an appointment?
If it is your first appointment, call 800-993-8244 to set up your patient credentials and schedule an available appointment time.

If you already have your patient credentials, you can schedule an appointment using the mobile app, the online patient portal by visiting careatc.com/patients, or by calling the healthcare center directly at 517-482-2420 during hours of operation. When calling after hours to make an appointment, call 800-993-8244.

What do I need for my first visit?

  • New patient paperwork: Please complete before your appointment at careatc.com/patient-forms
  • Valid ID
  • Medical insurance card

Who can use the SET SEG Family Healthcare Center?
All employees who have elected health coverage through eligible districts, their spouses, and their dependents are eligible to use the SET SEG Family Healthcare Center. Employees who waive the district’s coverage are not eligible to use the facility at this time. Your dependent’s information will already be loaded into the CareATC system. Scheduling an appointment can be done in the same way you schedule an appointment for yourself listed above.

How long do I have to wait to see the doctor?
The SET SEG Family Healthcare Center is designed so that you, your spouse, and your dependents will be able to schedule same-day and next-day appointments.

Do I have to make an appointment?
Yes. The SET SEG Family Healthcare Center only accepts patients by appointment. This allows for a patient-first approach that eliminates wait times and allows for extended visit times.

Services

What treatment is offered at the SET SEG Family Healthcare Center?

  • Primary care
  • Family medicine (newborns, children, adults)
  • Annual physicals and sports physicals
  • Acute illness (i.e. cold, flu, infection)
  • Immunizations
  • Osteopathic manipulative therapy (OMT)
  • Sprains and strains
  • Back or neck pain

Who will I see – the Registered Nurse, the physician, or the Medical Assistant?
Who you see  is dependent upon the reason for your visit. While the majority of primary and acute care visits will involve time spent with the physician, if you are going simply for a blood draw, you will likely only see the Registered Nurse or Medical Assistant.

Can I get my prescriptions filled at the SET SEG Family Healthcare Center?
The SET SEG Family Healthcare Center carries a variety of common generic prescription drugs that can be dispensed during a visit or to refill a prescription.

Does the SET SEG Family Healthcare Center provide labs and X-rays?
Yes.

Can my lab results be shared with a previously established primary care physician?
Yes. You, as the patient, would need to sign a records request form. Any records can be faxed to your primary care office of choice.

What if I injure myself and need a minor procedure?
The physician is able to perform minor medical procedures.

Cost

How much does it cost?
The cost of a visit depends on the plan in which you are enrolled and the reason for your visit:

Non-HSA plan:

  • $0 visits for preventive care (i.e. annual physical, PHA)
  • $0 visits for acute care (i.e. to treat an infection, injury, etc.)

HSA plan:

  • $0 visits for preventive care (i.e. annual physical, PHA)
  • $40 visits for acute care (i.e. to treat an infection, injury, etc.)
    • This fee is per visit and includes all services utilized during the visit including X-rays, lab work, and prescriptions.
    • $40 visit fee is applied to your deductible

Privacy

Will my employer have access to my health information?
No. The SET SEG Family Healthcare Center is operated by CareATC, an experienced healthcare provider that values patient privacy. As with all medical facilities, patient health records and personal information are kept confidential in accordance with strict laws and regulations.
